You can refer yourself to PeerLink, or someone else can make a referral for you.

If you’re a young person in Maine looking for peer support, or you know someone who may benefit from One-on-One Peer Support with PeerLink, starting a referral is the first step.

Who PeerLink May Be For

PeerLink may be available to Maine youth and young adults who:

  • Are between the ages of 14 and 26
  • Have a serious mental health diagnosis or identify as experiencing significant mental health challenges
  • Do not have insurance, or do not have insurance that covers PeerLink services

Not sure if someone is eligible? That’s okay. The referral process can help determine whether PeerLink is the right fit.

How to Refer Someone

Submit an Online Referral

After an online referral is submitted, Community Care will contact the referral source to complete the process. They’ll ask for some basic information so the PeerLink team can understand who is looking for support, where they are located, and what kind of connection may work best. This may include:

  • Referral date
  • Referral source and phone number
  • Referral agency, if applicable
  • How you heard about PeerLink
  • Young person’s name and date of birth
  • Address or county
  • Phone number
  • Preference for in-person or virtual support
  • Guardian name and phone number, if applicable

A Few Things Worth Knowing

PeerLink is peer-led and non-clinical. It is not therapy, medical treatment, case management, or a crisis service. PeerLink is built around shared experience, supportive relationships, and young people having a say in what support looks like for them.

You can refer yourself. You do not need a provider or another person to make the referral for you.
